Zenchess: White made several mistakes here. First, 9. d4 simply allows Black to occupy the e4 square and creates future problems. On move 16, either Nd4 or Qa4 were better than what he did; Nd4 is a discovery that allows the White knight to go to either c6 or e6, while Qa4 forces Black to misplace one of his pieces because it attacks the Knight. 20. Nh2 was a lemon that put the Knight out of play; 20. Nd2 followed by the exchange of Knights on e4 would have at least plugged up the long diagonal. Finally, 21. Bc3 was White's losing move.
