If the second exchange sacrifice is accepted, it's checkmate 21.BxRe2 Ng3#. This is called "Taimanov's Knight Check" by Murray Chandler, Deadly Checkmate #9 on pages 30-31 of "How to Beat Your (Anyone) At Chess". See the Diagram below:

This is a variation of Greco's Mate, where the knight is usually sacrificed to open the h-file for the queen (or rook) to deliver checkmate after the h-pawn is removed. But here, roles are reversed and the queen pins the h-pawn so the knight can deliver checkmate.
