May-29-16  Albanius: B pushing the g and h pawns exposed them.

After 49..Qxd7 50 Rxd7 Kxd7 the pawn ending is better for B.

Maybe W can hold, but his winning chances are nil except on a blunder. All W P's are isolated, passer is stopped by B K, and if W K tries to help th e-pawn, B gets an outside passed h pawn and eventually mops up on the Q side.

I suppose Caruana thought he had better winning chances holding onto Q, but the win on move 46 would have sufficed.
