Black is up a fairly significant pawn on c2. White gets off to a good start with <38.Nhxg6+ hxg6>, but then goes awry and misses 39.Nxg6+ Kg8 40.Ne7+. Now king moves lead to disaster, and after 40...Rxe7 41.dxe7 White's threat to promote leaves no time for a Black counter attack.

Instead, the game went <39.Qh4+ Bh5 40.Nxg6+ Kg8>. Now, White cannot go into 41.Ne7+ Rxe7 42.dxe7 Rxd5, as Black's bishop controls e8.

Hence, White has to pause for a moment and play <41.Qxh5>, which gives Black the move he needs for <41...Qb2>. After the continuation <42.Ne7+ Rxe7 43.dxe7 Qxc1+ 44.Kh2>:

It's thunderbolt time with <44...Qh1+>. If 45.Kg3 Qe1+, followed by 46...Qxe7 plucking the passed pawn while protecting the rook. And the text continuation leads to the same conclusion after <45.Kxh1 c1Q+ 46.Kh2 Qc7+>.
