  An Englishman: Good Evening: Soooooo...Black sacrifices the Exchange on move 13, and then--does *not* attack? Chess left me behind a long time ago.

12.Ne2 might appear a needed developing move, but it made the sacrifice possible. White might have had a higher priority in withdrawing the Bishop to b1.

Black seemed to do nothing for so long. Bet the silicon monsters went crazy trying to analyze a game with a sacrifice followed by no attack.

Disliked many of White's moves, incl. 18.Rg1, 21.a4 and 30.Nh5. The defense must have better choices somewhere.

So White must, but it is far from clear at a glance how he could have best improved his play.

True enough. In fact the easiest improvement to find is on the victor’s side:

With 39...Bg2+ 40.Kg1 Bh3, Black admits he’s gone wrong and if you look for that improvement it’s not hard to find. <39...Ng3+ 40. Kg1 Ne4> would have snuffed out all chance of counterattack and would have left White at Black’s mercy.

After that miss does White have a draw by perpetual/repetition? Fishie says yes:

It’s <44.Rxh3!>. Black has nothing better than 44...Rc1+ 45.Kg2 Rc2+ 46.Kg1 Rc1+ since <44...Qxh3?> would lose the Q to 45.Qxh7+ Kg4 46.Rg6+.
