E.g. it might work this way - an upload is done but the player's name is slightly different from the official db. So no dups are detected on upload, and the games get onboard thinking it's a new player. Later, Daniel's everything-and-the-kitchen-sink-too patch program <pgnfix> gets run - identifies the player's name is wrong (e.g. by using FIDE id), and fixes it. Viola - you get a heapful of dups. Of course you could argue it shouldn't be separate from the uploading, since the uploader so incorporate the <pgnfix> code. But I'd wager it still doesn't, and so this scenario could play out. .
