 | | tpstar: <WannaBe> Congratulations! White played 2. d4 to offer the Smith-Morra Gambit (1. e4 c5 2. d4 cxd4 3. c3) like the consultation game. One way to continue is 2 ... cxd4 3. c3 d5 Opening Explorer following the principle that ... d5 equalizes in the Sicilian. The primary drawback to 2 ... d6 is White may choose 3. dxc5 dxc5 4. Qxd8+ trading Queens and you can't castle. White should have grabbed the Pb7 with 6. Qxb7 followed by 7. Bb5 and castling up a Pawn. The early middlegame was fairly even, yet 16 ... Ra7!? & 17 ... Rfa8!? is a very unusual plan, especially without advancing the Queenside Pawns. You want your Rooks on e8 & d8, or c8 & d8, supporting your center. Instead of 28 ... Rb8 consider 28 ... a5 fixing your Pawns on dark squares where they can't be attacked by White's lsb. The endgame looked better for White yet 36. Bg4?? was a blunder losing to 36 ... Rd3+ (Discovery) and you won a piece. Keep practicing! =) |

 | | tpstar: <matey> The US government correctly identified four problems affecting all Americans: the high cost of health insurance, the millions of uninsured citizens, the unfair nature of insured folks paying higher premiums just to cover bad debt from the uninsured, and the ever-increasing costs of health care paid by the government itself. President Obama deserves credit for addressing these problems proactively, as politicians tend to react or overreact only, except his inexperience showed up at the worst possible time as this process has been the most mishandled government initiative I have ever seen, starting with the title. This was all about health insurance, not about health care per se, and I hate when bills are misnamed just to gain support ("No Child Left Behind" & "Patriot Act"). Moreover, it wasn't clear, it wasn't summarized, it wasn't bipartisan, it wasn't open or transparent or televised, and there is no excuse for holding important votes on holidays or Sundays just to ram things through. After their bipartisan meeting, it was clear they should have started over. Actually <HeMateMe> has the worst quote yet, from 11/23/09: "If the healthcare reforms go through, people will not allow personal taxes to skyrocket. Instead, hospitals and physicians will receive lower payments from the government. The highest earners in the healthcare profession, including the insurance firms, will make a little less money. They will have to accept anyone for treatment, as a requirement for staying in business/keeping a license to practice medicine. There may be a huge tug of war over this, but this country won't allow unfair taxation levels." The 20th century was the most successful to date for longevity, with US life expectancy increasing from 40-45 in 1900 to 70-75 in 2000 (depending what you measure). So in return, despite older and sicker and more complicated patients than ever before, doctors should accept less money to see more patients, and then have the government force unwanted patients on us. That approach is insane, and terming that approach "reform" is even more insane. Doctors' immediate concern is a 21.3% cut in Medicare payments which starts tomorrow 4/1/10 since Congress left without rescinding it. That will indeed save money, but anyone who thinks doctors will therefore accept more Medicare or even public option/government patients just doesn't understand Business. If any Medicare cuts go through. and this legislation isn't undone, expect a doctor strike in the next four years. |

 | | HeMateMe: <TP> I stand by my comment on this. the 4 problems you mentioned above are being combatted by this new legislation. Yes, its a bit fragmented, it is certainly a work in progress. So were social security and medicare. The Republicans wanted none of it, so the Democrats had to compromise, and pass the parts of this plan that they could. I'm sure there will be more students being accepted into medical school, if there is a real problem with doctors 'having to see more people.' I'm not sure how 21.3% cuts in medicare payments will affect the earnings of the typical physician. I guess it would depend on what percentage of your total earnings come from medicare payments. These premiums are out of control, and the uninsured cost the government a lot of money. If all of this somehow ends up in a shortage of physicians (highly unlikely) or a 'doctors strike' (even more unlikely), then the government will tweek this plan, and probably tax the highest earners a little more, so that doctors can keep their earnings at a level that doesn't decimate the medical industry. I thought some changes might occur at the medical school level. There are shortages of certain types of doctors, general practicioners, I think. There are also certain parts of the country that have a real shortage of doctors, as populations shift. Maybe this will be solved by requiring people in med school to work in a certain part of the country, for a certain number of years, as a requirement for getting a license to practice medicine. Shortages? Raise the taxes of the doctors who specialize in vanity medicines, like plastic surgery, liposuction, etc., and use that leverage to tax a GP less, in order to produce more doctors where there is a shortage. I would think the Democrats should accept a cap on lawsuit damages, which would lower the insurance premiums that doctors pay. These types of compromises and adjustments can make up for the lost income of physicians. At the end of the day, the doctors making the very most may lose a little bit of income, if they are in the top 5% of earners, making more than $250,000. There won't be many tears shed for anyone making that kind of money, in any industry, who has their taxes raised slightly. |
